1. We respect the personal data of our Patients, therefore we do not sell or make personal and address data available to third parties without the Patient's consent. Apart from the above case, the disclosure of the Patient's personal data may take place: - pursuant to a decision of an authority authorized by law, - in order to protect the rights of subject.
2. When using the website https://q-terapiamanualna.pl, the patient may be asked to provide some personal data by completing the form. We only require the data that is necessary for the proper operation of the portal.
III. What data do we collect?
1. Data collected automatically.
2. During a visit to the website, data regarding the Patient's visits are automatically collected, e.g. IP address, domain name, etc.
3. Data collected when the Customer contacts us.
When the Customer contacts us via the website, telephone, e-mail, etc., he provides us with his personal data, e.g. name, surname, e-mail address, home address for an outgoing visit, etc.
IV. How do we use patient data?
1. Under no circumstances do we sell data collected on the website to other entities.
2. The website collects information contained in system logs (date and time of connection, IP address and others). This data is used for technical, statistical and marketing purposes.
3. Data collected during correspondence between the Customer and the website will be used only to answer the Patient's questions.
4. The q-terpiamanualna.pl platform may use cookies to identify the Customer's browser when using the platform. Cookies do not contain any personal data.
5. In the event of a violation of the Regulations of our website, violation of the law, or when required by law, we may disclose the Patient's data to judicial authorities.
IV. e-mail
1. The owner reserves the right to send unannounced messages to people whose contact details he has and who have agreed to the Privacy Policy. Unannounced news should be understood as information relating directly to the company, industry websites, electronic publications, services, products (e.g. changes, internal promotions) and non-commercial letters (e.g. wishes).
2. Practical solutions and advice presented in electronic publications are not equally effective for everyone. Before using, all advice and tips provided by the portal should be analyzed in detail, compared with your own case, or possibly compared with other similar publications.
V. Change of data
1. The user has the right to correct his/her data.

Ladies and Gentlemen,
in accordance with art. 13 GDPR (General Data Protection Regulation) of April 27, 2016 (OJ EU L No. 119, page 1) I inform that:
1. The administrator of your personal data is Jakub Pietrzyk, running a business under the name: CERTUM Jakub Pietrzyk, NIP 677-221-59-79 address: Tetmajera 58 A2, 31-352 Kraków
2. Your data will not be transferred to a third country or international organization
3. Your personal data will be processed for the purposes of:
1) providing services, handling orders and responding to orders,
2) "Sensitive" data, i.e. patient cards containing information about the Users' health condition, will be used only by the administrator or employees employed by him. They are not made available to third parties. They are used exclusively for the purpose of providing medical services in the field of physiotherapy and manual therapy. "Sensitive" data is collected in a safe place and stored for a period of 5 years from the patient's last visit.
4. Your personal data is processed by the Administrator based on:
a) art. 6 section 1 letter b and c and art. 9 section 2 letter h GDPR in connection with joke. 25 point 1
Act on patient rights and § 10 section 1 point 2 of the Regulation of the Ministry of Health
as part of proper registration and provision of health services;
b) art. 9 section 2 letter h GDPR in connection with joke. 24 section 1 of the Act on Patient Rights and the Regulation of the Ministry of Health as part of maintaining your medical records;
c) art. 6 section 1 letter b and f GDPR, as the so-called the legitimate interest of the administrator, which is contact with reminders about the visit, its postponement and information about the results of the consultation.
d) art. 6 section 1 letter b, c and f GDPR and in connection with Pursuant to the Act of September 29, 1994 on Accounting, we have the right to process your data also for the purpose of pursuing any claims arising from our business activities or defending against them, as well as for tax purposes.
5. Providing data is voluntary, but necessary to use the services provided by the Administrator, to receive an invoice or to exchange correspondence.
6. Personal data processed by the Administrator are not subject to processes based on automated decision-making, including profiling.
7. Your personal data will be processed by the Administrator for the time necessary to conduct business in accordance with the law and archived (until the consent is withdrawn or the basis expires).
8. Your personal data is processed on a voluntary basis. You have the right to access your data and request its rectification, modification, deletion or limitation of processing, to object to the processing, as well as the right to transfer data.
9. Your personal data may be transferred to subcontractors, in particular IT solution providers, solely for the purpose of achieving the purposes listed in point. 2;
10. You have the right to lodge a complaint regarding the processing of personal data by the Administrator to the supervisory authority.
